I had some after market mags that had followers that were too short and would move foward enough to not catch the bolt hold open on the last round. I replaced them with Ruger factory followers and it cured the problem as they were a little longer.

I had a similar problem on one of my John Masen mags. Turned out that it wasn't the followers, it was the metal tabs at the top of the mag. I bent these slightly upward to allow the follower to rise a bit higher, and the problem went away.
